Refeeding syndrome is an historical disease though many uncertainties still remain. Biological markers associated with clinical manifestations are required for diagnosing refeeding syndrome. A novel classification of denutrition is necessary and should include encountered mechanisms like chronic disease related malnutrition, acute illness induced nutritional risk. Hypocaloric and hypoprotidic nutrition concept, during the acute phase of severe illness, could benefit the malnourished patients based on the nutritional risk induced by acute illness. Autophagy gets inhibited during the acute starvation condition. Hypophosphatemia and increased mortality associated with refeeding in critically ill patients may be related to autophagy inhibition.
