The management of COPD (Chronic Obstructive Pulmonary Disease) patients is based on pharmacological treatments (inhaled bronchodilators +/- inhaled corticosteroids) and on non-pharmacological therapeutics such as pulmonary rehabilitation program, vaccines, smoking cessation. In case of chronic respiratory failure, long-term oxygen therapy and non-invasive ventilation are appropriate. Endoscopic lung volume reduction (one-way endobronchial valves) is indicated for patients with severe dyspnea secondary to significant thoracic distension (Total Lung Capacity TLC>100% and Residual volume RV>175%) and in the absence of exacerbations despite appropriate medications. Finally, trials are on-going concerning several biologics for patients with frequent exacerbations despite well-conducted treatment.
