Transcranial Doppler (TCD) is a non-invasive technique for estimating cerebral blood flow, and is increasingly used in paediatric critical care. Quick to learn, easy to use and user-friendly at the patient's bedside, it provides clinicians with useful information in real time, which can refine the assessment of cerebral haemodynamics, provide prognostic elements or lead to therapeutic interventions. Knowledge of (i) the neurovascular physiology of the child and the systemic factors influencing it, (ii) the technical principles of TCD, and (iii) normal and abnormal velocimetric profiles are a prerequisite for proper interpretation of TCD. The absence of recommendations and the lack of academic and practical training are a risk of inappropriate use. Clinical contextualisation of the use of TCD and its integration into a multimodal neuromonitoring approach are necessary for appropriate and relevant use.
